RACE II

Mais um artigo sobre manejo do paciente com FA crônica. Dessa vez o RACE-II quis testar a não-inferioridade da abordagem menos agressiva da FC no paciente com FA. Ao contrário do AFFIRM, que comparou controle da FC (mais estrito) X Ritmo, o RACE-II abre uma perspectiva menos intensiva no manejo da FC nesses pacientes.

 Excelente revisão publicada em fevereiro na Nature, sobre manejo de fluídos no paciente crítico. Conceitos fisiopatológicos bem fundamentados e que vão de encontro com vários paradigmas das práticas atuais de reposição hídrica. Cada vez mais a máxima, "Mais, nem sempre é melhor" se aplica aos cuidados do paciente crítico. Observem os próximos artigos.

Both dopamine and norepinephrine are recommended as first-line vasopressor agent
in the treatment of shock. There is a continuing controversy about whether one agent
is superior to the other.In this multicenter, randomized trial, we assigned patients with shock to receive either dopamine or norepinephrine as first-line vasopressor therapy to restore and maintain blood pressure.
